    Foundations Behavioral Health Services is a community-based behavioral health center serving Mercer County and surrounding areas. Established in 1975, we have been providing professional counseling and support services to local residents for over 40 years. In 2012, the former Gateway Outreach Center merged with Foundations Behavioral Health, allowing us to offer a wide range of mental health and substance use disorder treatment.
